APPLICATION FORM

A. RIGHTS OF THE DATA SUBJECT

Pursuant to Article 11 of the Law on the Protection of Personal Data No. 6698 (PDPL) and the relevant legislation, every individual has the right to apply to our Company, which acts as the data controller, regarding the following matters:

  • a) To learn whether personal data is being processed,
  • b) To request information if personal data has been processed,
  • c) To learn the purpose of processing personal data and whether it is used in accordance with this purpose,
  • ç) To know the third parties to whom personal data is transferred, domestically or abroad,
  • d) To request the correction of personal data if it has been processed incompletely or inaccurately,
  • e) To request the deletion or destruction of personal data within the scope of Article 7 of the PDPL,
  • f) To request that the operations carried out pursuant to subparagraphs (d) and (e) be notified to third parties to whom personal data has been transferred,
  • g) To object to the occurrence of a result against the individual by analyzing the processed data exclusively through automated systems,
  • ğ) To request compensation for damages in case of damage due to unlawful processing of personal data.

 

B. PROCEDURES AND PRINCIPLES FOR APPLYING TO THE DATA CONTROLLER

According to Article 13(1) of the Law and the Communiqué on the Principles and Procedures for Application to the Data Controller published in the Official Gazette dated 10.03.2018 and numbered 30356; applications to our Company (as the data controller) regarding these rights must be submitted using one of the following methods or other methods determined by the Personal Data Protection Board ("Board"):

The data subject may submit an application in accordance with Article 11 of the Law No. 6698 and the relevant legislation:

  • • In writing,
  • • With a secure electronic signature,
  • • Via Registered Electronic Mail (KEP) address,
  • • With a mobile signature,
  • • From the e-mail address previously notified to the data controller and registered in the data controller’s system.
  • The following information must be included in the application:
  • • Name, surname and if the application is in writing, signature,
  • • Turkish ID number for Turkish citizens; nationality, passport number or (if available) identity number for foreigners,
  • • Residential or business address for official notifications,
  • • If available, e-mail address, phone number and fax number for notifications,
  • • Subject of the request.

If available, information and documents related to the subject must be attached to the application.

For written applications, the date on which the document is served to the data controller or its representative will be considered as the application date.

For other application methods, the date on which the application reaches the data controller will be considered as the application date.

Depending on the nature of the request, your application will be concluded free of charge as soon as possible and no later than thirty (30) days. However, if the process requires an additional cost, a fee may be charged in accordance with the tariff determined by the Board.

Applications must be submitted by the data subject personally. Applications made on behalf of someone else can only be processed with the submission of a notarized power of attorney, provided that it includes the authority to request information within the scope of the PDPL. If there is any doubt about the identity of the applicant, the data controller may request confirmation information.
